EN GDPR - Explicit consent to the processing of health data

1.   Your personal data (including your health data) are processed by DKV Belgium NV ("DKV"), a company incorporated and existing under Belgian law, having its registered office at 25, Rue de Loxum, 1000 Brussels, Belgium, registered with the Crossroads Bank for Enterprises under company number 0414.858.607.

2.   DKV attaches great importance to your privacy and will process your personal data (including your health data) in accordance with the General Data Protection Regulation (EU) 2016/679 (the "GDPR") and any national legislation adopted pursuant to the GDPR. DKV will process your personal data in the capacity of data controller.

3.   Your personal data (including your health data) will be processed by DKV in accordance with the DKV Privacy Statement. The latest version of this statement can always be found at www.dkv.be/privacy.

4.   Prior to concluding an insurance contract, DKV processes your health data in response to medical questionnaires/requests specifically with a view to assessing risks and eligibility for the conclusion of the insurance contract. During the execution of the insurance contract (and possibly after) DKV processes your health data for the management, processing and execution of your claims, including for the prevention, detection and investigation of insurance fraud. 5.   In certain cases and to the extent strictly necessary for the execution of the insurance contract or the provision of our services to you, we will share your health data with a limited number of third parties (located mainly in the EEA). 6.   DKV processes your health data inter alia by automated means which may have legal consequences for you (e.g. to establish the pricing, to determine the conditions of acceptance and the extent of the insurance coverage, claims settlement, etc.).

7.   Your health data can only be processed with your explicit consent. If you do not give your explicit consent, the conclusion and/or proper execution of the insurance contract may be hindered.

8.   Once given, you have the right to withdraw your consent at any time. However, the withdrawal of consent does not affect the lawfulness of the processing based on the consent prior to its withdrawal. The withdrawal of your consent may again impede the proper performance of the insurance contract.
